What began in the fall of 2017 as an Integrated Youth Services initiative has grown into a comprehensive, trusted network supporting youth and families across Guelph and Wellington County. Built on the vision of a system where health and social services work side by side and where young people can “walk through one door” to find the support they need this initiative has evolved into The Grove Youth Wellness Hubs, and now Youth Wellness Hubs Ontario (YWHO) – Wellington Guelph.
Today, YWHO – Wellington Guelph serves the community through five youth wellness hub sites and two satellite locations, offering accessible, coordinated care for young people and their families.
Over the past year alone, youth accessed services nearly 40,000 times, reflecting both a growing demand for integrated supports and the trust young people place in this model of care.
In the coming months, the community will begin to see a gradual transition in branding from The Grove Youth Wellness Hubs to Youth Wellness Hubs Ontario – Wellington Guelph. This evolution aligns the local network with a province‑wide system of more than 32 Youth Wellness Hub networks across Ontario and reflects its role within this broader, connected framework of care.
Each Youth Wellness Hub works closely with local service providers and community partners to integrate existing services in a coordinated, youth‑centred way. The goal is to ensure that youth aged 12 to 25 can access free, high‑quality supports, including mental health and substance use services, housing supports, education and employment assistance, and peer support either within their own community or at any Youth Wellness Hub across the province.
Aligning local branding with the provincial network helps young people clearly identify where support is available and builds confidence that, should they move to another Ontario community, they will encounter familiar and consistent care. The network has already seen the positive impact of this connectivity for youth transitioning between communities.
As part of this transition, community members will begin to notice updated signage and email domain changes effective May 1. While the name and visual identity are evolving, the commitment behind the work remains unchanged. Youth Wellness Hubs Ontario – Wellington Guelph continues to provide barrier‑free, integrated services and remains dedicated to transforming Ontario’s health and social services system with and for youth to improve outcomes that matter most to young people and their families.
